Inity CCPRX Home Care LLC, provides physician-nurse clinic-home visits on Monday's and Fridays, and case management daily, which allows those that cannot get to a doctor to be cared for. Providing access to traditional and alternative medicine, healthy living, and support has helped our patients stay out of the hospital, and nursing homes. Case management assists with accessing community resources. We provide wound, catheter, IV care, and are available when needed.
Home visits from Dr. Wilson Family Practice-Gerontology, nursing, and case management provided by Nia Becker RNBSN, are having a positive effect on people in the underserved communities, those home-bound with chronic co-morbidities.
The most gratifying part for me is to see improvements in physical, mental health and the empowerment of our patients to live healthier and navigate the health care system successfully.
