I attended University of Michigan Medical school and residency at UCSF.
Fees are 600 for a 2 hour eval FTF and 150-250 for follow ups.
My office is at 6001 Butler Ln Scotts Valley CA 95066.
First visits are in person and follow up is either remote or in person.
I do not accept insurance but can provide paperwork for you to send in your bill.
