NOTICE TO OUR PATIENTS: 1-10- 2024
To our dear patients:
It is with mixed emotions that I am announcing my retirement from active adult practice for patients older than 22 years old effective February 17, 2024. It has been a great pleasure providing your familys allergy, asthma, and immunodeficiency care needs over the past eight years. Providing patient-centered, high-quality adult allergy and asthma care is one aspect of my practice that is difficult for me to give it up.

As of February 19, 2024, I will be working at the Pediatric Allergy-Immunology Clinic at the Texas Children's Hospital-Austin, first at the Mopac Specialty Care Center, then at the North Campus where I will continue to see patients from birth up to age 22 years of age.

Your medical records are privileged and confidential; a copy can be transferred to: 1 my new clinic for patients up to 22 years old, 2 released to you, or 3 released to another Allergist-Immunologist you designate with your written permission. If your child will continue to see me, you can sign an authorization form to release her or his or their files to Texas Childrens Hospital Austin as soon as you receive this notice by contacting Texas Childrens Hospital Mopac Specialty Center Allergy-Immunology Clinic at 737 220-8200. If you are over 22 years old, you will need to choose a different allergist from a recommended list available at our office. Please sign the authorization forms at our office and return it to us on or before February 16, 2024 so we may transfer your records and treatment vials to your new allergist-immunologist. Until then, your records will remain on file at this clinic for the next forty-five 45 days.
