Office entrance is on the outside of Costco just to the right of the main doors.
A Costco membership is NOT required for an eye exam!
Eye exam services: routine exams, glasses exams, contact lens exams, diabetic exams, and medical eye exams, including red eye visits, eye infections, dry eye evaluations, and cataract and glaucoma evaluations.
Our offices uses Optos retina photos to evaluate the health of the retina.
Emergency eye visits and same-day appointments are available. Please call our office if there is no open slot on the schedule.

Insurances accepted:
We accept vision plans, medical, medicaid and medicare insurance. Please specify your insurance in the "Reason for Visit field" when booking online, if possible, so we can verify coverage.
Medical: Most insurances including Medicare, Tenncare, Aetna, BCBS, Cigna, UHC, UMR, Ambetter
Vision: Most insurances including VSP, Davis, Superior, Alwayscare, Avesis, Envolve, NVA, March, Spectera, Premier, some Eyemed plans
Note: Eyemed does not allow us to take all plans, however, you can use your out-of-network benefits and get reimbursed by Eyemed.

_____TO BOOK AN APPOINTMENT FOR AN EYE EXAM_____
1. Click on a time. Use the arrows to select a later date.
2. Enter your information. Please specify your medical and vision insurance in the Reason for Visit field
3. Click on the blue arrow on the right to advance to the next screen to confirm.
4. If you have having trouble booking online, please call or text 615-379-9881
Please note: the website will not allow you to book a child appointment, but you can put the parent or guardian's name in the Date of Birth field and put the child's date of birth in the "Reason for Visit" field to proceed to the next step.
Walk-ins available, but appointments are recommended.
If you are having EYE PAIN, REDNESS, or CHANGES IN VISION, please call or text us at 615 379-9881. If you are having a medical emergency and unable to reach the doctor, please head to nearest emergency room.
