Please use this to coordinate scheduling online appointments for patients being discharged from the hospital. Please use the appointment notes section when scheduling to specify the following:

Discharge Date
Type of appointment request: PsychMedicalBoth
Patient ins and Member ID
Patient address and Phone Number
Name of Case Manager Scheduling Appointment

Appointments will be transferred to preferred provider or first available provider and the patient will be notified

Patients will be scheduled with next available providers unless specified provider is listed in the appointment note
Walk in patients welcome 9 am to 4 pm daily!
IF THE PATIENT DOES NOT HAVE A PHONE NUMBER, PLEASE SEND THEM TO THE CLINIC AS A SAME DAY WALK IN, WE ARE HAPPY TO ASSIST WITH TRANSPORTATION WHEN THE NEED ARISES
